From the moment of admission, caring professionals from Nursing, Dietary, Activities, Social Services and rehabilitation departments begin to assess each resident’s unique needs. The team meets as a group and begins to formulate a plan of care – which may include assistance from other healthcare professionals, clergy, support organizations or family members.
Family members are invited to attend personal conferences to discuss, change and modify, when appropriate, the prescribed plan of care.
